FACILITY EVALUATION REPORT

The annual case management inspection identified three deficiencies related to staff training compliance. Specifically, the facility failed to provide documentation for annual routine medical visits and failed to keep pre-admission appraisals updated for certain residents.

The inspection identified two deficiencies related to staff training and resident care documentation. Specifically, staff training records were deficient regarding postural supports, and physician's orders were missing for half bed rails on two clients. Both deficiencies are cited as Type B, requiring corrective action by July 1, 2024.
