The investigation substantiated allegations of neglect concerning client #1. The primary deficiency cited is a failure to provide basic supervision when the client was left unattended in a vehicle. This incident posed an immediate health and safety risk to the resident in care.
Basic services were not provided because staff failed to supervise client #1 on 7/22/25 when the facility driver exited the van, leaving the client and private caregiver inside without supervision.
