What this means for your family

While some families report positive outcomes for short-term rehab, the recurring reports of medication errors and hygiene neglect are serious red flags. If you consider this facility, we strongly recommend unannounced visits and asking for a detailed plan on how they manage medication schedules and prevent resident wandering.

While some residents experience positive rehabilitation outcomes and praise individual staff members, a persistent pattern of understaffing and administrative failures leads many families to strongly advise against choosing this facility.

Key Review Excerpts

My Mother came in and found him in wet pants and soaked sheets more than once. He was given only 1shower in a week. He is an alzheimers patient and twice got completely out of the facility and was found more than a mile away!!!

Memory care family member · 2018☆☆☆☆

The care is inconsistent and inferior due to lack of employed staff and the overuse of temporary agency help. I witnessed degrading treatment of my Father by a temp CNA the day before he died.

Long-term resident's family · 2019☆☆☆☆

My mother was there after a major surgery. Her incision became infected after the wound care nurse dropping a cloth in the sink my mother washed her hands and brush in teeth and cleaned her incision with it.

3 complaint-triggered

This facility shows persistent safety and regulatory issues across multiple areas. Fire safety violations appear repeatedly across all surveys, including problems with sprinkler systems, exit signs, and emergency equipment. Medication management and infection control have also been cited multiple times. Families have filed complaints that triggered inspections, resulting in findings about accident prevention and resident communication. While all deficiencies show correction dates, the recurring pattern of similar violations suggests ongoing compliance challenges that families should discuss during visits.
