The Acknowledgement of Inspection form was signed and left at the facility for each date of the inspection. The licensee has the opportunity to submit a plan of correction to indicate how the cited violation(s) will be addressed in order to return the facility to compliance and maintain future compliance with applicable standard(s) or law. If the licensee wishes to provide a plan of correction: (i) type the plan on a separate Word document, (ii) identify the standard violation number being addressed, (iii) include the date the violation will be corrected, (IV) do not include any names or confidential information, and (V) return to the licensing inspector by email within five (5) business days of the exit interview. Compliance with all applicable regulations and law shall be maintained and any areas of noncompliance must be corrected. Within 15 calendar days of your receipt of the inspection findings (inspection summary, violation notice, and supplemental information), you may request a review and discussion of these findings with the inspector's immediate supervisor. To make a request for review and discussion, you must contact the licensing supervisor at the regional licensing office that serves your geographical area. Evidence: 1) A review of the record for resident #1 contained documentation showing a sex offender registry search for resident #1 was completed on January 11, 2026. 2) Resident #1 was admitted to the facility on April 08, 2025.
Based on record reviews, it was determined that the provider did not ensure that all new employees received training on all the required topics outlined under 22VAC40-73-120.C within 7 days of being hired. Evidence: 1) A review of the respective employee records for employee #1 and employee #2 did not contain documentation showing these employees were trained on the following required topics: a. Use of the first aid kit and knowledge of its location b. Handwashing techniques, standard precautions, infection risk-reduction behavior, and other infection control measures specified in 22VAC40-73- 100 c. Requirements and procedures for detecting and reporting suspected abuse, neglect, or exploitation of residents and for mandated reporters, the consequences for failing to make a required report, as set out in ? 63.2- 1606 of the Code of Virginia d. Based on record reviews, it was determined that the provider did not ensure that all direct care staff employees have current certification in first aid training from the American Red Cross, American Heart Association, National Safety Council, American Safety and Health Institute, community college, hospital, volunteer rescue squad, or fire department. To be considered current, first aid certification from community colleges, hospitals, volunteer rescue squads, or fire departments shall have been issued within the past three years. It is the last phase of the initial licensure process. An inspection of the proposed location, to include the interior and exterior of the home, and surrounding grounds, was completed as part of the process. Each room designated as a resident bedroom was measured to ensure the room met the required square footage for the number of residents intending to occupy the room. The windows in each of these rooms was measured for the same purpose. After reviewing the room and window requirements, it was determined that only one room met the requirements for dual occupancy. The other rooms designated as bedrooms can only accommodate one resident. The evidence gathered during the inspection determined no violations with applicable standard(s) or law. The inspection summary will be posted to the VDSS website should the facility be issued a license to operate. The department's inspection findings are subject to public disclosure.
