Failed to implement COVID-19 pandemic policies, resulting in missing risk assessments for community outings for 7 of 7 sampled clients and lack of daily screening for some clients.
Failed to transfer funds within required timeframes for three non-sampled clients who left the program (one in nursing home, two deceased).
Failed to provide specific instructions for a PRN bowel medication, leading to staff confusion on when to administer it.
Failed to ensure medication organizers were filled only by authorized parties; staff were physically assisting a client with filling their medi-set.
Staff assistance with medication refills and containers was not consistent with WAC requirements.
Failed to ensure a safe environment; egress windows in a client's bedroom were painted shut and inoperable.
The agency was unable to close client accounts due to bank refusal and freezing of accounts based on FDIC regulations.
