Key Review Excerpts

One of the nurses discovered my sister unresponsive. The nurse immediately called emergency services and she was transported to ORMC hospital. The E/R staff stabilized my sister. The quick action probably saved my sister's life.

Long-term resident's family · 2023★★★★

Unfortunately care was not good across the board and there were other times when I walked in to find him soiled with no one around to help change him. My mother and I cleaned him up and changed him several times ourselves while waiting for people to help after pushing the call light.

Short-term rehab family member · 2023★★★☆☆

They ordered medication when we brought his medication from home and the nursing staff genuinely did not care and didn’t give him his eye drops for glaucoma which is very painful for him.

Families filed complaints about safety hazards, leading to two deficiency reports, with the same accident prevention issue cited in both 2023 and 2025 surveys indicating a persistent safety concern. The facility shows recurring problems across resident assessment and care planning, quality of care including nutrition and respiratory services, and infection control. While all deficiencies have reported correction dates, the repeated safety violations and broad range of care issues across multiple surveys suggest ongoing operational challenges that warrant careful consideration during any visit.
