What this means for your family

This facility's physical therapy department is highly regarded and often results in successful recovery for short-term patients. However, families should be extremely cautious regarding long-term care, as multiple reviewers have reported consistent issues with neglect, slow response times, and poor communication. We strongly recommend conducting unannounced visits and closely monitoring your loved one's daily care if you choose this facility.

Key Review Excerpts

My mother arrived at Greenwood not able to walk and by the time she left she was able to walk 50 or more feet with a special walker. Kudos to Bet and Dan from Therapy.

Long-term resident's family · 2025★★☆☆☆

She was constantly told that a person would be there to help her but they constantly “forgot” she even needed help. There’s no way they just forgot, because there are loud buzzers signifying that a patient needs help. Instead, often times, they would simply stand around and ignore patients.

Memory care family member · 2024☆☆☆☆

My grandmother was in a rehabilitation facility to gain strength because she could not stand or walk. She was supposed to be there for 20 days but came home early due to poor care. She needed to use the bathroom and ended up spending two hours on the toilet, which put strain on her spine.

Greenwood Center has significant ongoing issues across multiple surveys, with families filing 15 complaints that triggered federal investigations. The facility repeatedly struggles with basic care quality, infection control, and fire safety violations. Most concerning, five recent fire safety and electrical deficiencies from January 2026 remain uncorrected with no improvement plan. While the facility corrects most violations when cited, the pattern of recurring problems in resident care, safety standards, and building maintenance suggests persistent operational challenges that families should carefully consider before placement.
