The facility failed to ensure the building and structure complied with the 2000 edition of the Life Safety Code, NFPA 101.

The facility failed to provide reasonable access and/or necessary equipment for maintenance, testing, and servicing of equipment.

The facility failed to provide at least one telephone for use in an emergency, and/or that emergency numbers were posted at or near the telephone.

The facility failed to document that staff were competent and trained prior to assuming their responsibilities.

The facility failed to ensure the required sprinkler system was inspected, tested, and maintained in compliance with NFPA 25.

The facility failed to ensure the building was kept in good repair.

The facility failed to have a COVID-19 response plan that includes plans for obtaining and maintaining a two-week supply of PPE.

The facility failed to ensure the fire alarm was always visible to staff or is monitored by a device always carried by the staff.

The facility failed to ensure lighting levels were in compliance with licensing standards for assisted living facilities and the Illumination Engineering Society of North America.

The facility failed to ensure interior walls and/or ceilings were constructed with a material having at least a 20-minute fire rating, or at least 3/8 gypsum board.

The facility failed to provide resident toilet and bathing facilities in compliance with licensing standards for assisted living facilities.

The facility failed to ensure resident bedroom doors were in compliance with licensing standards for assisted living facilities.

The facility failed to provide the required sprinkler system to meet NFPA 13, 13D, 13R requirements, or meet the requirements of Chapter 32.
