What this means for your family

This facility is highly regarded for its physical therapy and on-site dialysis, making it a strong candidate for short-term rehab. However, families should be extremely cautious regarding communication and weekend staffing; we recommend conducting an unannounced visit and specifically asking how the facility handles medication management and call-bell response times.

Key Review Excerpts

I came in this week for physical therapy. So far everything has been great. I am in a brand new renovated room, the room and the rest of the building looks very nice.

Rehab patient · 2024★★★★★

My father was hospitalized 2 times because of the lack care, he was supposed to have therapy but they kept avoiding him. On June 5th he sent to the hospital cause of a heart attack, the workers didn’t help him they just made matters worse.

Long-term resident's family · 2025☆☆☆☆

The onsite dialysis is AMAZING here! They use Nextstage machines that help patients recover within an hour avoiding Fatigue.

Recent complaints led to investigations revealing concerns about resident rights and abuse reporting, which are serious red flags. The facility shows ongoing issues across safety, care planning, and fire protection systems that have persisted across multiple surveys over five years. While all deficiencies have correction dates, the pattern of recurring problems in safety systems and the complaint-triggered findings about potential abuse suggest families should carefully evaluate this facility.
