What this means for your family

This facility offers a strong rehabilitation program and a welcoming environment, but the consistency of care is a major concern. Families should conduct unannounced visits, specifically during weekends or evenings, to observe staffing levels and hygiene standards before committing to a stay.

Key Review Excerpts

My mother just spent roughly two weeks at this facility. While the building itself is old and a little drab, the majority of the care she got was excellent. Her PT and OT were phenomenal and exactly what she needed.

Rehab patient's family · 2025★★★★

My husband was in this facility for 1 week in July of 2025. This facility is filthy, the smell was horrific. I went in everyday and had to clean his area. His roommate was there with a broken hip and I had to empty his urinal nobody would do it.

Long-term resident's family · 2025☆☆☆☆

Although there are a few excellent staff members here, the medical care for patients with severe medical needs is lacking. Most of the nursing staff are temps so they are not aware of the patient history.

Franklin Wellness has recurring issues across multiple care areas, with families filing complaint reports that triggered recent inspections. The facility shows persistent problems with medication management, resident assessment and care planning, and safety hazards including accident prevention and fire safety systems. While all deficiencies show provider correction dates, the pattern of repeated violations in medication handling, care planning, and safety protocols across surveys from 2019 to 2025 suggests ongoing operational challenges that families should discuss during visits.
