This report details the findings of a Complaint Investigation conducted on March 17, 2026, regarding allegations of abuse. All three allegations—sexual abuse, physical assault, and being yelled at—were determined to be unsubstantiated based on interviews and record reviews.

The facility underwent an unannounced Case Management visit regarding a missing person incident. The Licensing Program Analyst interviewed staff and the resident, and reviewed the physician's report. No deficiencies were cited during this inspection.

The inspection identified three deficiencies, all classified as Type B. Issues noted include inadequate hallway temperature, lack of window coverings in a resident's room, and insufficient hallway lighting. All deficiencies require corrective action by specified due dates.

Two Type B deficiencies were cited concerning the requirement for continuous on-site supervision and the availability of timely on-call staff.
