This report details a Complaint Investigation conducted on 05/20/2025 regarding allegations of inadequate food service and poor record keeping. The investigation found that both allegations were unsubstantiated based on staff interviews and facility observations.

The inspection identified several deficiencies across multiple areas, including personnel record keeping, expired administrative certification, and emergency preparedness. Specifically, there are multiple Type A citations related to immediate risks, such as expired administrator credentials and deficiencies in personnel files. Several Type B deficiencies were noted concerning emergency supplies and resident care planning.

Key areas of concern include outdated fire extinguisher inspection records, improper storage of chemicals, lack of current CPR/First Aid training among staff, and improper storage of perishable food items like eggs. The facility underwent a Pre-Licensing evaluation on December 5, 2022. While many areas were found adequate, two deficiencies were noted: missing required posters and the absence of grab bars in resident areas. These issues require corrective action to achieve full compliance.
