What this means for your family

While the facility has a dedicated social work and therapy team that many families praise, the recurring reports of slow call-bell response times and medication errors are critical red flags. If you choose this facility, we strongly advise that you or a family member visit daily to monitor care, as the quality of service appears to be highly dependent on individual staff members rather than consistent facility-wide standards.

Google Reviews

Google Reviews

234 reviews on Google
Fair Oaks Health & Rehabilitation receives highly polarized feedback, with many reviewers praising specific staff members—particularly in social services and physical therapy—while others report severe neglect and safety concerns. While some families describe a 'gold standard' of care, a significant number of reports cite long wait times for call bells, poor hygiene, and medication errors. Potential families should be aware that experiences appear highly inconsistent and dependent on the specific unit or staff assigned.

Key Review Excerpts

The staff is very helpful, and insures that residents are cared for. You can tell that the administration and nursing staff take pride in the work they do, and treat the residents like family.

Professional partner · 2024★★★★★

The facility is very old and not very maintained. I give this facility one star because there was truly gem in there, Jackie. If it wasn’t for her our stay would’ve been horrible.

Family member · 2024☆☆☆☆

My mom was a patient at this dump and it's a dirty place. The pictures are lies and the so called employees are worthless. I highly recommend not going to this place.
