COMPLAINT INVESTIGATION REPORT

The inspection identified multiple deficiencies across several critical areas, including fire safety, emergency systems, and recordkeeping. Specifically, there are multiple Type A violations related to fire clearance and outdated alarm systems, alongside several Type B violations concerning incomplete personnel and resident records, and lack of required staff training and emergency drills. Immediate corrective action is required for the Type A violations.

The inspection report details findings from two separate complaint investigations conducted on or around August 13-14, 2025. The first investigation found the allegation regarding staff training to be Substantiated, citing deficiencies in medication training records. The second investigation concerning pest infestation and cleanliness was deemed Unsubstantiated. The only formal deficiency cited relates to staff medication training compliance.

This report details a complaint investigation conducted regarding allegations of staff misconduct, inadequate food service, drug activity, and financial abuse. All allegations reviewed were deemed unsubstantiated based on staff and resident interviews and record review. No deficiencies were cited in the final report.

The facility was visited for a required annual inspection. The Licensing Program Analyst conducted interviews with staff and residents and performed a health and safety tour. No deficiencies were cited during this visit, and the LPA plans to return at a later date to continue the annual inspection.
