The inspection was conducted to review the facility's annual compliance. Three deficiencies were cited, all of which are Type B violations related to incomplete resident records. Specifically, the facility was found to be missing required Pre-Admission Appraisals, Functional Capabilities Assessments, and a required annual reappraisal for a resident with Dementia.

The unannounced case management visit on September 26, 2024, was conducted by LPAs Kiran Jain and Komal Charitra. The visit was to deliver an amended report related to an Annual inspection conducted on September 11, 2024. No deficiencies were issued during this visit.

Two other deficiencies are Type B, concerning staff health screening documentation and failure to conduct required emergency drills.

Additionally, there were Type B deficiencies concerning the adequacy of staff living accommodations and privacy. The facility was required to submit several updated forms and plans of correction by August 12, 2022.

The inspection found no immediate health risks, but several areas require follow-up and improvement. Deficiencies noted include the need for additional signage regarding infection control practices, and the administrator restricted access to staff files. Furthermore, the facility must submit several updated administrative and operational forms by a specified deadline.
