The provider failed to implement the Client's Individual Instruction and Support Plan (IISP) regarding goals and community inclusion activities, such as exercise, cooking, and researching Special Olympics.
The provider failed to treat the client with dignity and respect. Staff occupied the living area with personal items, spoke in an undignified manner about the client in their presence, and failed to access available trust funds to purchase items that would enhance the client's quality of life.
