A relicensure survey was completed on 12/9/25. Deficiencies were cited. Based on interview and record review the residence failed to ensure there was at least one staff member onsite at all times with current certification in cardiopulmonary resuscitation (CPR) from a nationally recognized organization and shall include a skills assessment observed and evaluated by an instructor affecting 66 current residents. Findings include:On 12/9/25 at 8:50 a.m., staff CPR certifications and the 11/16/25-12/13/25 staff schedule were provided by the administrator.The residence' s staff schedule and CPR certifications revealed that the residence failed to ensure that there was at least one staff member with a current certification in cardiopulmonary resuscitation (CPR) from a nationally recognized organization that included a skills assessment observed and evaluated by an instructor on 11/2.. Based on interview and record review the residence failed to, on a quarterly basis, audit the accuracy and completeness of medication administration records (MARs), affecting 66 current residents.Findings include:On 12/10/25 at 10:45 a.m., the last two quarterly medication audits were provided. However, the quarterly medication cart audits revealed no evidence that the administrator participated in the audits. On 12/10/25 at 2:25 p.m., the administrator stated that she does not participate in medication administration records, controlled substance list, medication error reports, and medication disposal records quarterly audits. Based on observation and interview, the residence failed to place in a visible location a list of all staff who havecurrent certification in first aid or cardiopulmonary resuscitation (CPR) so that the information is readily availableto staff at all times, affecting 66 current residents.Findings include:During the onsite visit on 12/9/25, the residence failed to have a list of staff members with current certification infirst aid and CPR in a visible location and readily available to staff at all times throughout the residence.On 12/9/25 at 1:55 p.m., the administrator stated that she expected the list of CPR certified staff be updated and hanging in the staff room, she acknowledged the list had not been updated and was not posted in the residence at the time of the onsite visit. THIS PORTION OF THE REPORT IS FOR INFORMATIONAL PURPOSES ONLY.No response is necessary.The residence was advised it must review and maintain the following processes in accordance with existingprogram regulations found at 6 CCR 1011-1, Chapter 7.13.8 Before the next regularly scheduled meeting, assisted living residence staff shall respond in writing to any suggestions or issues raised at the prior meeting.
