The Acknowledgement of Inspection form was signed and left at the facility for each date of the inspection. A complaint was received by VDSS Division of Licensing on January 26, 2026, regarding allegations in the area(s) of: PERSONNEL, STAFFING AND SUPERVISION, RESIDENT CARE AND RELATED SERVICES, RESIDENT ACCOMMODATIONS AND RELATED PROVISIONS, BUILDINGS AND GROUNDS The licensing inspector completed a tour of the physical plant that included the building and grounds of the facility. Number of interviews conducted with residents: Number of interviews conducted with staff: 3 Observations by licensing inspector: The tour of the facility did not reveal unsafe or unsanitary conditions. An inspection of the medication carts contained accurate documentation that reflected the controlled medications for applicable residents. The medication carts were locked and secured. Residents were noted to be appropriately dressed and groomed for the time of day, weather conditions, and activities they were engaged in. Additional Comments/Discussion: Discussed with the administrator, director or nursing, and director of resident services the methods used by the facility to ensure residents who require frequent checks (i.e. incontinent residents) are checked and changed. The facility uses a charting system that is signed off by staff at the required intervals for those residents who require frequent examinations. The same type of charting system is used to ensure resident?s apartments are inspected regularly and that garbage is removed timely to prevent infestations and/or foul odors from developing. The evidence gathered during the investigation did not support the allegation(s) of non-compliance with standard(s) or law. A complaint was received by VDSS Division of Licensing on December 12, 2025, regarding allegations in the area(s) of: ADMINISTRATION AND ADMINISTRATIVE SERVICES, RESIDENT CARE AND RELATED SERVICES Number of interviews conducted with staff: 2 Additional Comments/Discussion: Prior to the complaint being received, the provider notified the Department of an gastrointestinal outbreak affecting several residents. The provider reported they were coordinating with the local Virginia Department of Health and implementing their infection prevention policies and procedures. The provider?s infection prevention policies and procedures were reviewed. It was also determined that the infection was contained to the assisted living wing of the facility, and no residents who reside in the memory care of the wing were infected. It was also determined that while residents were encourage to quarantine in their rooms, they were not prohibited from leaving their rooms. This allowed them continual access to all areas of the facility, to include hydration stations. In addition, those residents who were asked to quarantine, had their meals brough to them. These meal deliveries, in addition to routine checks by staff, and medication administration, afforded multiple staff members at multiple times of the day to assess residents needs. The evidence gathered during the investigation did not support the allegation(s) of non-compliance with standard(s) or law. It shows the resident identified in the complaint appearing to be asleep in their wheelchair in a common area of the facility. Another resident appeared to trip on the wheelchair of the resident identified in the complaint, causing them to loose their balance and fall on the resident identified in the complaint, causing the resident identified in the complaint to be dislodged from their chair. The video footage shows the incident appears to be accidental. The evidence gathered during the investigation did not support the allegation(s) of non-compliance with standard(s) or law. Type of inspection: Complaint Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: An on-site inspection of the facility occurred on May 12, 2025 from approximately 11:45 AM ? 2:00 PM. The Acknowledgement of Inspection form was signed and left at the facility for each date of the inspection. A complaint was received by VDSS Division of Licensing on March 26, 2025, regarding allegations in the area(s) of: STAFFING AND SUPERVISION Number of interviews conducted with staff: 1 Observations by licensing inspector: Residents were appropriately dressed and groomed for the activity they were engaged in and for the weather conditions and time of day. Interactions amongst residents and with staff were appropriate. Additional Comments/Discussion: The facility has implemented appropriate communication systems amongst management and direct care staff to ensure staff for each shift are aware of issues that may have arisen from the previous shift. The facility conducts rounds on the secure unit every two hours in addition to have a call-alert system for resident assistance. A review of staffing schedules for the previous three months shows that the facility meets the staffing requirements for staff to resident ratios. Number of residents present at the facility at the beginning of the inspection: 59 The licensing inspector completed a tour of the physical plant that included the building and grounds of the facility. Number of resident records reviewed: 6 Number of staff records reviewed: 3 Number of interviews conducted with residents: Number of interviews conducted with staff: 2 Observations by licensing inspector: Additional Comments/Discussion: An exit meeting will be conducted to review the inspection findings. The evidence gathered during the inspection determined non-compliance with applicable standard(s) or law, and violation(s) were documented on the violation notice issued to the facility. The licensee has the opportunity to submit a plan of correction to indicate how the cited violation(s) will be addressed in order to return the facility to compliance and maintain future compliance with applicable standard(s) or law. If the licensee wishes to provide a plan of correction: (i) type the plan on a separate Word document, (ii) identify the standard violation number being addressed, (iii) include the date the violation will be corrected, (IV) do not include any names or confidential information, and (V) return to the licensing inspector by email within five (5) business days of the exit interview.
