Type of inspection: Renewal Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: Two on-site inspections were completed as part of the renewal process. Record reviews were conducted on October 21, 2025, between approximately 11:00 AM and 4:15 PM. An inspection of the facility and surrounding grounds was completed on November 07, 2025. The Acknowledgement of Inspection form was signed and left at the facility for each date of the inspection. Number of residents present at the facility at the beginning of the inspection: 124 The licensing inspector completed a tour of the physical plant that included the building and grounds of the facility. Number of resident records reviewed: 8 Number of staff records reviewed: 4 Number of interviews conducted with staff: 4 Observations by licensing inspector: Residents were observed interacting with each other and with staff. These interactions were appropriate. The residents observed were appropriately dressed and groomed for the time of day and the activities the residents were engaged in. Residents were also observed engaged in self-directed and staff led activities. There were no health or safety citations. Two of the four elevators in the building were inoperable. The facility has contracted with a service provider to ensure these two elevators are operational as soon as possible. The evidence gathered during the inspection determined non-compliance with applicable standard(s) or law, and violation(s) were documented on the violation notice issued to the facility. The licensee has the opportunity to submit a plan of correction to indicate how the cited violation(s) will be addressed in order to return the facility to compliance and maintain future compliance with applicable standard(s) or law. If the licensee wishes to provide a plan of correction: (i) type the plan on a separate Word document, (ii) identify the standard violation number being addressed, (iii) include the date the violation will be corrected, (IV) do not include any names or confidential information, and (V) return to the licensing inspector by email within five (5) business days of the exit interview. Compliance with all applicable regulations and law shall be maintained and any areas of noncompliance must be corrected. Within 15 calendar days of your receipt of the inspection findings (inspection summary, violation notice, and supplemental information), you may request a review and discussion of these findings with the inspector's immediate supervisor. To make a request for review and discussion, you must contact the licensing supervisor at the regional licensing office that serves your geographical area. Regardless of whether a supervisory review has been requested, the results of the inspection will be posted to the DSS public website within 5 business days of your receipt of the Inspection Summary and/ or Violation Notice. Based on a review of documentation and interviews, the facility did not ensure that a new resident agreement that meets the specifications of 22VAC40-73-390 was signed by those residents who chose to remain at the facility after a change of ownership occurred. Evidence: 1) A new license reflecting the change in ownership was issued to the facility on June 01, 2025 2) A review of the respective records for resident #2 and resident #5 did not contain a new resident agreement between the residents and the new licensee that meets the specifications of 22VAC40-73-390. 3) Staff #1 confirmed that a new resident agreement between resident #2 and resident #5 and the new licensee that meets the specifications of 22VAC40-73-390 was not completed.

Based on a review of documentation and inter-views, the facility did not ensure that each staff person, on or within seven days prior to their first day of work at the facility, and prior to coming into contact with residents, submitted the results of a risk assessment documenting the absence of tuberculosis in a communicable form. Evidence: 1) A review of the record for staff #3 did not contain a risk assessment documenting the absence of tuberculosis in a communicable form. 2) Staff #3 was hired on September 12, 2024. 3) A risk assessment was located in the record for staff #3, however, the risk assessment was not dated, so it could not be determined when the screening occurred. 4) Staff #1 confirmed a completed, signed, and dated risk assessment was not in the record for staff #3

Based on a review of documentation and interviews, the facility did not maintain documentation of monthly emergency generator tests. Evidence: 1) The facility is equipped with an on-site emergency generator and is subject to the requirements of conducting and documenting monthly tests of the emergency generator. At the time of the on-site review there was only one documented test of the generator.
