The facility failed to ensure medications were administered as ordered by a licensed prescribing practitioner. Specifically, for one resident, there was no Vitamin C 250 mg available for administration despite an active order for the dosage. Instead, the facility was using a house stock of 500 mg Vitamin C.

The facility failed to conduct a criminal background check prior to the hire of one sampled employee (Staff A). This occurred following the staff member's transfer to the Assisted Living facility.

The facility failed to ensure that two of six sampled staff members (Staff C and Staff F) had a Health Care Personnel Registry (HCPR) check completed prior to their employment. Documentation was missing for both employees, and the staff members were unaware that such checks were required.

The facility failed to ensure that 2 of 6 sampled staff members (Staff C and Staff F) had no substantiated findings listed on the North Carolina Health Care Personnel Registry (HCPR) prior to their employment. Personnel records for both the cook and the Activity Director lacked documentation of completed HCPR checks.
