What this means for your family
This facility is physically well-maintained and offers a strong therapy program, which is a significant plus for recovery. However, families should be aware of potential gaps in management responsiveness and patient advocacy; we strongly recommend meeting with the nursing director to discuss how they handle care coordination for residents with cognitive needs.

40 reviews on Google“City Park Healthcare and Rehabilitation Center receives high praise for its physical environment, therapy services, and overall atmosphere from many visitors and some residents. However, there is a notable discrepancy between these positive reviews and critical feedback from a resident regarding management responsiveness, medication delays, and inconsistent quality of care among staff. Families should be aware that while the facility is physically well-maintained, experiences with daily care management and advocacy appear to vary significantly.”

Key Review Excerpts
“While no facility is perfect, or "just like home", our experience with City Park has been the best so far. We find the staff to be very professional, warm and caring.”
“The building is clean. While there are certain employees here that work very hard to provide good service to residents the overall experience is not good for me because of those who do not. I deal with unresponsive and argumentative management, delays with receiving needed medication and therapy and other necessities.”
“While everyone is friendly and helpful, their team approach leaves no one actually managing the patient which is a disaster when the patient is geriatric with mental deficits. Advocacy was pretty much ignored.”

A complaint survey, prompted by #CO34868 and Incident #CO35052 was completed on 3/13/24 to 3/18/24. One deficiency was cited.
