What this means for your family

While the facility offers specialized medical equipment, the recent and consistent reports of neglect, poor hygiene, and slow response times are significant red flags. If you are considering this facility, I strongly recommend conducting an unannounced visit to observe staff responsiveness and cleanliness firsthand, and asking specifically how they address concerns regarding resident hygiene.

Key Review Excerpts

This facility offers a family based atmosphere with knowledgeable staff who advocate for their residents. The difference between this facility and all the others I have been is the genuine amount of love the staff have for their residents and their families.

Family member · 2023★★★★★

My mother can hit her call button and she has to wait and wait and wait until somebody finally comes in to see what she wants not only that some of the aids are very rude

Long-term resident's family · 2023★★☆☆☆

This place smells so bad of black mold! I have told the works each and every time I have been there with my husband. Last few times we have gone, we meaning my husband, myself and his two aunt have had to get her dressed because she was still laying in bed with night clothing on, un brushed hair, etc.

$179,235 in fines

Circle of Care shows persistent problems across 69 deficiencies over six surveys, including five complaint-triggered issues where families filed reports. The facility has recurring problems with infection control (appearing in multiple surveys), medication management, and care planning. Recent complaints in 2024-2025 involved medication practices and failure to notify families of resident conditions. While all deficiencies show correction dates, the pattern of repeated issues in core care areas warrants careful consideration.
