Questions for Your Tour

  • 1Given that physical therapy is a noted strength here, could you explain how your rehab team coordinates with daily nursing staff to ensure residents maintain their progress?
  • 2I noticed some feedback regarding response times for assistance; what specific protocols or technology do you have in place to ensure call lights are answered promptly?
  • 3With the current staffing levels, how do you ensure that residents receive consistent support with daily hygiene and room cleanliness?

Key Review Excerpts

Answering call lights in a reasonable time is a big issue. As is adhereing to scheduled 'shower' days. However, I ALWAYS was able to communicate with staff regarding my husband's needs/complaints to get issues resolved.

Long-term resident's family · 2022★★★★

A few nice nurses and STNAS. The care was questionable. Request for medical things like a wheelchair with no tread on the wheels. The socks was never gotten for him. Request were made to the DON. They messed up on his medication not good.

Family member · 2025☆☆☆☆

The care and services he received....absolutely wonderful! They worked with him daily in physical therapy, took him to doctors appointments he had, made sure he had everything he needed.

3 complaint-triggered

This facility has received three complaint-triggered inspections where families reported concerns about accident prevention and resident money management. The most recurring problems involve fire safety issues, quality of care (including pressure ulcer care and daily living assistance), and nutrition services. While all deficiencies show correction dates, the pattern of fire safety violations appearing across multiple surveys and family complaints about safety and financial management suggests ongoing operational challenges that merit careful evaluation during your visit.
