The facility failed to ensure that the resident's physician authorizes personal care services and certifies the care plan by signing and dating it within 15 calendar days of the assessment completion.

Specifically, two oxygen cylinders were found on the floor in front of a storage rack in room 116. Staff indicated that the cylinders were placed on the floor because there was no other available storage space.

The facility failed to ensure that hot foods were maintained at 135 degrees Fahrenheit or higher until residents were ready to eat. Observations during breakfast and lunch services showed food being served from dishes placed on top of a warmer rather than being held at the required temperature.

The facility failed to perform timely follow-up on medication aide verification for one of two sampled employees. This resulted in a failure to verify the required training and competency status prior to the employee working on the medication cart.
