The facility administrator stated they were in the process of getting trained to be a trainer following the departure of the previous staff trainer.
Failed to ensure 75-hour training certificates/exemption letters for three of four sampled staff and failed to ensure four of four sampled staff completed twelve hours of CE in 2023.
Failed to verify accuracy of reconciliations of checking, cash, and personal funds for four of four clients in the sample for January 2024 through May 2024.
Failed to ensure three of four sampled staff kept first-aid/CPR certification current.

Follow-up inspection on 06/05/2024 confirmed that WAC 388-101D-0295-1-a, WAC 388-101D-0295-1-b, and WAC 388-101D-0295-1 were corrected.
Provider failed to ensure a medication system was in place to properly provide medications as prescribed, resulting in errors on two separate occasions.
