Provider failed to maintain a current running balance for Client 8's funds and debit card transactions.
Provider failed to ensure proper health service supports for Client 3, specifically regarding bowel-movement protocols, medication administration instructions, monitoring prescribed medical treatments, and medical follow-ups.
Provider failed to ensure safety requirements for 3 of 8 sampled clients. Issues included Client 4 lacking a working phone for 2 months, and water temperatures exceeding 120°F in the homes of Client 6 and Client 8.
