Specifically, there have been instances of missed medication doses and inaccurate records indicating medication was given when it was actually unavailable.

Medication administration errors: Resident A missed doses of oxybutynin on 12/15/24, 12/16/24, and 12/17/24; staff documented administration of medication that was not physically in the building. Resident B missed multiple doses of multivitamin and Vitamin D3; staff documented administration of Vitamin D3 on 1/6/25 when the medication was unavailable; facility failed to notify family timely regarding the need for vitamins.
Employee 1's file did not contain evidence that initial TB testing was completed within the required 10-day timeframe of hire (hired 9/27/24, test completed 5/4/24).
Resident A moved into the facility on 12/12/23 but her TB screen was not completed until after she moved in on 1/4/24.
