The facility failed to ensure that the preparation and administration of medications and treatments were in accordance with physician orders and facility policies. This included a need for retraining medication aides on the 6 rights of medication administration and improving the tracking of new physician orders.

The facility failed to maintain the kitchen, dining, and food storage areas in a clean and orderly condition. Specific issues included grease and food buildup on the stove, oven, and fryer, as well as dirt, dust, and grime on the refrigerator, freezer, ice machine, and air conditioning unit. Additionally, sinks and door handles were found to be dirty with visible food particles and liquid drips.
