Medication administration errors: 1) Missing signatures on narcotic counts for medication cart #1 during shift changes between 4/11/2025 and 4/29/2025. 2) Blank entries for Haloperidol and Lorazepam doses for Resident A on 3/27/2025. 3) Staff administered Metoprolol Tartrate to Resident F on multiple dates (including 3/3/2025, 3/4/2025, etc.) despite vital signs being outside the prescribed parameters (systolic BP below 110 and/or pulse below 65).
