What this means for your family

This facility has a highly regarded therapy team that can be instrumental in recovery, but families should be prepared to be very hands-on. You must actively manage the discharge process and follow up daily with case management, as communication and administrative follow-through are the most significant weaknesses identified by multiple families.

Boswell Transitional Care of Cascadia receives polarized feedback, with many patients praising the therapy and nursing staff for their dedication and effectiveness in recovery. However, significant concerns exist regarding administrative follow-through, poor communication from case management, and inconsistent quality of food and facility cleanliness. Families should be aware that while clinical outcomes are often reported as positive, the discharge planning and social services departments are frequent sources of frustration.

Key Review Excerpts

My stay at Boswell Transitional was a great experience. I have stayed at many rehab facilities and I will say that this one is my favorite. I needed the OT and PT desperately to get back to where I was prior to being sick.

Rehab patient · 2025★★★★★

The nurses are nice as are most of the CNAs, but I am beyond disgusted with the case worker and social worker at this facility. Expecting them to do the jobs they get paid to do is like pulling teeth.

Family member · 2024☆☆☆☆

My 94 yr old mom who has Alzheimer’s is there and on the 12th day I still hadn’t heard from the Doctor. I found out that the doctors don’t talk to the family until you request it through the nurses.
