What this means for your family

This facility offers a modern, clean environment and strong rehabilitation therapy, which many families appreciate. However, given the recurring reports of theft and serious lapses in clinical oversight, we strongly recommend that families conduct frequent, unannounced visits and maintain active involvement in their loved one's medical oversight.

Questions for Your Tour

  • 1Given the facility's recent health inspection results, what specific steps is the leadership team taking to improve clinical oversight and ensure consistent quality of care?
  • 2I noticed some feedback regarding communication challenges; what is the best way for family members to reach the nursing team or administration to get timely updates on their loved one?
  • 3With the facility having a 4-star staffing rating, how do you ensure that same level of support and responsiveness remains consistent during the weekends?

Key Review Excerpts

My father went in fine, but when we came to pick him up to move him to a smaller facility he had open wounds on his legs. The staff that was "caring" for him did not even bring him out.

Long-term resident's family · 2022☆☆☆☆

In less than a couple of weeks of being there, she was sepsis and delirious. My aunt, upon realizing her wound was worse and the severity of the neglect, had to then DEMAND that the facility release her.

Memory care family member · 2025☆☆☆☆

While my mother-in-law was placed in this facility December of 2022 and had her wedding ring stolen by one of the staff. My wife reported this to the home director who said she would file a police report; according to the WPD, she did not, so we did.
