What this means for your family
While the facility is physically beautiful and offers strong rehabilitation services, there is a recurring pattern of neglect regarding basic needs and slow response times to call lights. If you choose this facility, ensure you have a plan for frequent family visits to monitor care and advocate for your loved one's needs, as staffing levels appear inconsistent.

While many reviewers praise the facility's appearance and the dedication of specific staff members during rehabilitation stays, a significant number of families report serious concerns regarding neglect, slow response times to call lights, and poor communication. Key Review Excerpts
“My father was a resident from 11/2015 until he passed away on 5/27/19. The care and compassion from the staff is exceptional. There is no such thing as perfection, however, this facility and the people employed there are as close as it can get.”
“My mom had fall risk and no bed alarms.. she fell 12 times!! They had a mat next to her bed that was removed every time we went to see her then they they would put it down just while we were there. Finally she fell broke her knee, sent to hospital came back to avenue.. fell again then had to have surgery!!”
“I waited for assistance in the bathroom. I waited 90 minutes with no help.”
