The facility failed to serve therapeutic diets as ordered by the physician for one resident with a reduced concentrated sweets (RCS) diet. Specifically, the resident was offered a chocolate chip cookie for dessert on 04/06/22, which contradicted the dietary restrictions noted in the menu and physician orders.

The facility failed to ensure that one of two sampled medication aides had completed the required state-approved medication administration training or provided verification of previous employment. Specifically, records for Staff C did not demonstrate completion of the necessary 5, 10, or 15-hour training courses required prior to administering medications.

The facility failed to ensure that one of three surveyed staff members (a Medication Aide) had completed the required annual state infection control training. Personnel records showed the staff member's last training was completed in July 2015, and no subsequent training was documented.
