What this means for your family

While the facility offers a robust activities program and supportive social services, the recurring reports of pest infestations and severe hygiene neglect are major red flags. If you are considering this facility, we strongly recommend conducting an unannounced visit to inspect the cleanliness of the rooms and speaking directly with current residents' families about their experiences with medication administration.

Worcester Rehabilitation & Health Care Center receives highly polarized feedback, with many families reporting severe neglect, unsanitary conditions including pest infestations, and poor communication. While some residents and family members praise specific staff members—particularly in the activities and ambassador programs—these positive experiences are frequently overshadowed by reports of delayed medication, unaddressed call lights, and a lack of basic hygiene for patients.

Key Review Excerpts

My mother in law currently stays here. And god what a nightmare it is every time we come to visit. It’s sad how run down the facility is. ... the buildings infested with mice, she can’t even order groceries for herself without mice getting into them

Memory care family member · 2025☆☆☆☆

The staff does not care about the patients. My dad is a current patient there. He has been left in his chair for long periods of time when he cannot sit. It will take them 30 to 45 minutes to change his diaper which is uncalled for and has caused him sores.

Long-term resident's family · 2021☆☆☆☆

The highlight here is the recreation director jeniffer vale. She is always there with a smile and is very ingagingshe and her staff have made a activity packed calender.

$81,776 in fines

Families filed multiple complaint reports that led to deficiency findings, including serious concerns about neglect and abuse. The facility has recurring problems with resident rights (particularly safe and comfortable living conditions), environmental safety issues like call systems and pest control, and medication management. While most violations show planned correction dates, the pattern of repeated issues across multiple surveys - especially the same environmental safety problems - suggests ongoing challenges with maintaining consistent care standards.
