A relicensure survey was completed on 10/24/24. Deficiencies were cited. Based on observation and interview, the residence failed to ensure the secure outdoor area was available year-round and independently accessible to residents without staff assistance for entrance or exit, affecting 16 current residents. Findings include:On 10/23/24 at approximately 8:00 a.m., the door to the secured courtyard was locked, denying access for the residents. A test of the door revealed two locks on the door, one showed it needed to be physically locked by simply turning the padlock. The outdoor secured area was inaccessible to residents who tried to access the secured area without the assistance of staff. On 10/23/24, Staff #2 stated the door automatically locked when closed. Staff #2 also stated the night crew locked both locks to the courtyard at night, denying access to the residents. Staff .. Based on observation and interview, the residence failed to have a means of securing resident records that preserved their confidentiality and provided protection from loss, damage, and unauthorized access, affecting 16 current residents.Findings include:On 10/23/24 at approximately 7:30 a.m. to approximately 12:00 p.m., an environmental tour revealed the residence had their resident records office door propped open. During this time, residents, family members, along with caregivers from external service providers were in the halls with access to the propped open room. On 10/23/24 at approximately 5:00 p.m., the administrator stated the office door was to be closed and locked at all times of the day and night. She stated the resident records were not secured in the room when the door was pr.. Based on observation and interview, the residence failed to provide a physically safe and sanitary environment, affecting 16 current residents.Findings include:On 10/23/24 at approximately 8:00 a.m., the exhaust hood for the gas stove in the common kitchen was observed to have no grease trap. This resulted in the exhaust hood being saturated with cooking grease, which was a potential fire hazard. On 10/23/24, at approximately 5:00 p.m., the administrator stated she was not aware there was anything missing from the exhaust hood. She also acknowledged it was potentially a fire hazard. THIS PORTION OF THE REPORT IS FOR INFORMATIONAL PURPOSES ONLY.No response is necessary.The residence was advised it must review and maintain the following processes in accordance with existing program regulations found at 6 CCR 1011-1, Chapter 7.14.30 The assisted living residence shall maintain a record on a separate sheet for each resident receiving a controlled substance which contains the name of the controlled substance, strength and dosage, date and time administered, resident name, name of authorized practitioner, and the quantity of the controlled substance remaining.
