An initial licensure survey was completed on 7/24/24. A deficiency was cited.The residence requested licensure as a secure environment to serve residents with cognitive impairment. Based on observation and interviews, the residence failed to be in compliance with all applicable regulations. This failure created the potential for mismanagement of the care of and services for the residents who would be served by this residence.Findings include:-21.1 The assisted living residence grounds shall be kept free of high weeds, garbage, and rubbish.On 7/23/24 from approximately 1:30 p.m. to 2:45 p.m., there were collated framing nails lying on the ground on the north side of the residence. On the south side of the residence at the base of the brick fence wall, there was a gap between the fence and the concrete pad that contained landscape/gravel rock. There were various pieces of wood and bricks scattered between the edge of the concrete walking pad and the wall itself. The administrator acknowledged the observations.-21.2 The assisted living residence grounds shall be maintained to protect residents from slopes, holes or other hazards.On 7/23/24 from approximately 1:30 p.m. to 2:45 p.m., the secure outdoor area contained the following potential tripping hazards:Approximately one foot of metal edging was exposed. The ground surrounding a sprinkler valve box had a difference in surface depth of approximately one-half to one inch. There was a change in surface depth of approximately one-half inch square in the concrete pad. Under the .. The assisted living residence and staff shall observe these rights in the care, treatment, and oversight of the residents. The statement of rights shall include, at a minimum, the following items: (A) The right to privacy.
