The facility failed to administer medications as ordered and in accordance with facility policies for one resident. Specifically, a medication aide administered only one spray of Flonase per nostril instead of the ordered two sprays and crushed a Mirabegron ER tablet, which is an extended-release medication that must not be crushed.

During a medication pass on 11/08/22, a medication aide administered only one spray of Flonase in each nostril for a resident instead of the ordered two sprays. This resulted in a medication error rate of 24% during the observed morning medication pass.

This failure resulted in one resident suffering heat stroke, sunburn, and dehydration after being found unresponsive outside, and another resident experiencing multiple falls.
