The most recent renewal inspection from May 2024 identified several concerns regarding resident service plans, specifically regarding the lack of physician orders for assistive devices and insufficient detail for medication administration. Additionally, the facility had issues with invalid admission contracts and improperly secured oxygen tanks.

Resident C had bedside assistive devices attached to his bed without physician orders; the service plan lacked purpose of use, staff responsibility, maintenance schedules, and monitoring instructions.
Admission contracts for Residents A, B, C, and D were invalid because they were between Baruch SLS, Inc (d/b/a Baruch Senior Ministries) and Union Court Assisted Living, whereas the licensee is listed as DCS Management LLC.
Resident B's medication administration record (MAR) for Haloperidol lacked detailed information in the service plan regarding how anxiety is demonstrated, specific behaviors requiring medication, or the use of nonpharmaceutical interventions. Similar findings noted for Residents A and C.
Oxygen tanks were stored in resident rooms and were not safety secured, posing a risk of harm.
