Hazardous cleaning agent was found unsecured underneath the kitchen sink cabinet, posing an ingestion risk.
A mop bucket with water was found in the kitchen, creating a potential hazard to food safety.
The laundry room was being used for storage (bed frames, wheelchairs) and contained a mop bucket instead of being a dedicated clean/soiled linen area.
Tables in the dining room were peeling and unsanitary, presenting a potential hazard.
Employees did not know the procedures for assisting residents in the event of a fire.
