What this means for your family

This facility presents significant safety and communication risks that should not be ignored. If you choose this facility, you must verify their protocols for patient supervision and ensure you have a reliable way to communicate with your loved one's caregivers.

Key Review Excerpts

The food and activities are very good, The hallway and rooms are neat and the best in cleaness. The facility is over 40 Years old, need some repairs and update, They are working on it, The best feature is the Staff,

Long-term resident · 2016★★★★

And TWO OF THE PATIENTS WALKED OUT OF THE FRONT DOOR WITHOUT ANY NURSE NOTICING! I went in there as more of the patients were trying to get out and I shouted “two of your patients just walked out” and they just kept staring, gossiping with each other .

Family member · 2018☆☆☆☆

Patients get best care when there is an inspection going on or the Board comes. Lots of food deliveries come, but residents never see most of the "good stuff", especially the meat deliveries (except hot dogs)! Personal items of residents go missing a lot.
