Key Findings
A special investigation in March 2024 identified serious concerns regarding a resident not receiving timely medical follow-up for injuries that resulted in broken ribs, as well as improper use of medical equipment by staff. The original 2007 licensing study showed the facility was in substantial compliance.

Direct care worker was observed by relatives using a spirometer incorrectly (the opposite way) while assisting Resident A.
Resident A did not receive appropriate follow up treatment for an unexplained bruise to the eye and chest pain; specifically, the resident was not seen by the physician on the requested date, and an emergency department visit later revealed two broken ribs.
