Based on record review and interview, the manager failed to ensure that a caregiver provided evidence of freedom from infectious tuberculosis on or before the date the individual began providing services at or on behalf of the assisted living facility. Findings include: 1. A review of E3's personnel record revealed a chest X-ray, negative for radiographic evidence of acute or chronic tuberculosis, and a Risk Assessment and Signs and Symptoms Screening Form. Further review revealed no documentation of a 2-step TB Test or a Blood Test. 2. In an interview, E1 acknowledged that there was no documentation of a 2-Step TB Test or Blood Test.
