What this means for your family

This facility is highly regarded for its physical therapy and rehab outcomes, making it a strong candidate for short-term recovery. However, families should be aware of inconsistent cleanliness and night-shift responsiveness; we recommend conducting a thorough tour of the specific room your loved one will occupy and asking about the facility's plan for addressing after-hours care needs.

Key Review Excerpts

With a couple of exceptions (will address later), the staff on the Rehab Ward could not have been more caring & competent. I'm the type to spend LONG hours in a facility when she's there, it's just my non trusting nature, but two of her nurses, Carrie & Bonnie made me comfortable enough to actually "take more

The aides did their part on helping me get healthy by helping get clean when I needed it, helping me become more independent, and just attending to all my basic needs. The nurses also did a great job at making sure I got my medications on time.

14deficiencies
Above state avg (10.1)
6 complaint-triggered

This facility shows concerning patterns of recurring deficiencies across multiple areas, particularly fire safety systems, medication management, and care planning. The most troubling signal is that families have filed complaints triggering additional inspections that found issues with accident prevention, medication errors, and proper care delivery. While the facility has addressed each individual deficiency, the repeated problems with fire safety equipment (sprinkler systems, smoke detectors, exit doors) across all surveys and ongoing medication management issues suggest systemic challenges that families should carefully evaluate before choosing this facility.
