The facility failed to ensure a functional assessment was completed within 30 days of admission for Resident #3. The assessment should have determined the resident's level of functioning, including psychosocial well-being, cognitive status, and physical functioning in activities of daily living.

The facility failed to ensure medication was administered in accordance with physician orders for one resident. Specifically, the resident's prescribed dose of Ativan was not administered as ordered between August 14, 2021, and August 24, 2021.
