The facility failed to maintain plumbing equipment in a safe and operating condition, as evidenced by a leaking toilet in the shared bathroom for rooms 210 and 212. This leak resulted in standing black water on the bathroom floor and had been an ongoing issue for months to years according to resident and staff interviews.
The facility failed to ensure personal care assistance for a resident who required assistance with toenail care. A review of body evaluations revealed that staff were not consistently addressing nail cleanliness, smoothness, and length as required by the resident's care needs.
