This lack of oversight prevented the resident from receiving necessary mental health treatment for depression and neurocognitive disorder.

The resident was also unable to participate effectively in virtual visits due to improper use of equipment.

The facility failed to ensure that electronic medication administration records (eMARs) were accurate for a resident regarding the documentation of medications administered by injections. Specifically, there was a lack of updated physician orders for fingerstick blood sugar checks and Humalog insulin sliding scale parameters.

Specifically, there was no designated area to document the units of Humalog insulin administered per the sliding scale orders, and for many instances, there was no documentation of the units administered despite blood sugar readings requiring insulin.

The facility failed to ensure food storage areas were protected from contamination due to the presence of rodent droppings in the dry goods storage area. Specifically, droppings were found on windowsills, storage containers for sugar and flour, and on an empty shelf. Additionally, cleaning logs were incomplete, and the last recorded pest control treatment was in October 2019.

The facility failed to maintain the premises in a clean and hazard-free manner. Specific issues included an active bed bug infestation in multiple resident rooms, saturated and deteriorating ceilings, and accumulated dirt and debris on floors and in bathrooms. Additionally, foul-smelling standing water was observed in a janitor's closet sink.
