What this means for your family

The facility is highly regarded for its smooth admissions process and helpful administrative staff, making it a strong choice for initial placement. However, families should be vigilant regarding daily nursing care; we recommend conducting unannounced visits and asking specific questions about how the facility monitors hygiene and prevents bedsores, as these have been recurring concerns.

Google Reviews

Google Reviews

83 reviews analyzed
The Bluffs Rehabilitation and Healthcare Center receives highly polarized feedback, with many families praising the admissions process and specific staff members like Celeste Norris for their kindness and efficiency. However, there are serious, recurring allegations from residents and family members regarding neglect, poor hygiene, disrespectful staff behavior, and difficulty reaching the facility by phone. Prospective families should weigh the strong administrative support during intake against significant concerns regarding daily nursing care and responsiveness.

Key Review Excerpts

The entire staff has been knowledgeable and extremely helpful. Leaving a loved one is a daunting task. The staff at The Bluffs let us leave in the knowledge that she will be well cared for.

Long-term resident's family · 2025★★★★★

I would like to commend Michelle Wooten for her observation and quick response to the needs of my brother in law when he returned to the facility after being discharged from the hospital. As a result of her commitment to her clients, my brother in law was able to get the treatment he needed and is now on his way to recovery.

Family member · 2025★★★★★

This facility has significant quality concerns with multiple families filing complaints that triggered 10 of the 26 total deficiencies found across five surveys. The most recurring problems involve care planning, resident safety and protection from harm, and communication with families about transfers and bed holds. While the facility has corrected most issues, the pattern of repeated deficiencies in care planning and the concerning number of complaint-driven investigations suggest ongoing challenges with basic care standards.
