The facility underwent an unannounced annual inspection on July 1, 2024. The Licensing Program Analyst reviewed resident and staff files, and toured all areas of the facility. No health or safety violations were observed during the inspection.

The annual inspection identified two primary deficiencies. The most critical finding (Type A) relates to the unauthorized use of the facility garage as staff bedrooms, which poses a fire safety risk. Additionally, a Type B deficiency was noted regarding incomplete staff health records, specifically missing TB test results in multiple staff files.

The facility underwent an unannounced annual visit. The Licensing Program Analyst found no immediate health, safety, or personal rights violations during the tour of common areas, resident bedrooms, and restrooms. The facility was found to be in substantial compliance at the time of the inspection.

The complaint investigation determined that the allegation was substantiated. The primary deficiency cited relates to admitting a resident with a prohibited health condition (gastrostomy tube) in violation of CCR 87615(a)(2). The facility acknowledged the violation and provided a plan of correction.
