Key Review Excerpts

My mother was in there and almost died for three days she was complaining of severe pain. They told her that she was fine and her vitals were fine so the third day my sister and I called an ambulance which ticked off Donna one of the nurses and she told me that’s on you, not me I outrank any EMT and there’s nothing wrong with her.

Long-term resident's family · 2024☆☆☆☆

Within 2 weeks, he was up on his feet walking several steps and feeding himself again. The administrator and nursing team met with me individually to make sure his needs were met as we expected.

Rehab patient's family · 2024★★★★★

My 93 year old neighbor was sent here for rehab after breaking her hip. They're SEVERELY understaffed. Call lights on for hours. No answer at the main number most of the time.

Families filed reports leading to 12 complaint-triggered deficiencies, including serious concerns about resident protection from abuse and neglect that prompted a recent December 2024 inspection. The facility has recurring problems with accident prevention and safety supervision, care planning, and daily living assistance that have persisted across multiple surveys from 2021 to 2024. While all deficiencies show correction dates, the pattern of repeated issues in core care areas suggests ongoing quality challenges.
