A special investigation was conducted regarding allegations of improper care and bruising of a resident, for which a corrective action plan was approved. A subsequent renewal inspection identified issues with missing signatures on assessment plans and incomplete weight records, all of which were addressed via approved corrective action plans.

Specifically, hospice workers observed major bruising to Resident A's abdominal area and believed staff member Todd Patrick used excessive force when transferring the resident.
